Assess patient needs for diabetes education and provide diabetes self-management training to inpatients at Halifax Health Medical Center (Daytona Beach and Port Orange campuses). Serve as a professional resource person regarding diabetes for fellow staff members, physicians/providers and the community. Graduate of an accredited nursing program. Registered Nurse with bachelor’s degree. Certified Diabetes Care and Education Specialist (CDCES) or willingness to work towards CDCES certification is preferred. Required to have a minimum of three years of clinical experience. Acceptable areas of experience include acute care setting, physician office setting, home health care, inpatient or outpatient diabetes education services. Experience in working with diabetes patients required. Prefer Certified Diabetes Care and Education Specialist or willingness to work towards it. The educator will be responsible for providing diabetes education for inpatients at both the main Daytona Beach hospital as well as Port Orange hospital.
